Name; date admitted; age; birthplace; occupation; religion; ship of arrival; how long in colony; place of residence; marital status; place of marriage, at what age, and name of spouse; names and ages of children living; number and sex of children deceased; father's name and occupation; father's present residence if living (or 'father dead'); mother's maiden name; disease or reason for admission; date of discharge or date and cause of death; sometimes additional remarks (medical history, social circumstances etc.)
Note that ship of arrival, place of residence, marital status and father's present residence are NOT shown on a Queensland death certificate; and a hospital register is sometimes the only surviving source of information about immigration.

WEBSTER, Leeds, c1833
I am seeking references to James Porter WEBSTER, born about 1833 in Leeds (West Yorkshire).
James Porter WEBSTER was the son of George WEBSTER and Sarah (nee GIBLETT). I was surprised to find (in a census) that James was born in Yorkshire, as his family was from London, Middlesex and Surrey. I suspect that George's work may have involved some travelling!

Tombstone Tuesday - Mary AGAR, 1794

In 2002 I took this photograph of a headstone in St. Aidan's churchyard, Gillamoor, North Yorkshire. It is for my 4x-great-grandmother, Mary AGAR. It reads:
"...Mary AGAR the wife of Jonathan AGAR who died Jany 27th 1794 aged 47 years".
Associated surnames in the areas of Farndale, Lastingham and Kirbymoorside include CLARK, PEACOCK and MERCER or MARCER.

I am researching Peter MATTHEW or MATHEW (sometimes written as MATTHEWS or MATHEWS) of Barton-le-Willows (parish of Crambe, North Yorkshire). He married Catherine (Kitty) ASHTON in 1803 at Kirby Misperton.
Their daughter Christiana MATTHEW (1806-1892) married William HUDSON of Barton-le-Willows. Another daughter, Jane MATTHEW, married William LEAPER.
